are many ways to make an investment in the future of healthcare
in our community. One way is to make a gift to the INI Foundation at OSF Saint Francis Medical Center Foundation. Through your charitable gift,
you may realize a tax deduction, but more importantly, you'll be
joining others who share in the same Mission to provide
excellent care to people who are sick regardless
of their circumstances in life.
Every donation makes a difference in the lives of our patients and their families. Your gift will help us:

Checks/Credit Cards/Cash
To make a
gift by postal mail, please print out the gift form and
mail or fax it to INI Foundation, 530
NE Glen Oak Avenue, Peoria IL 61637 or fax your credit card gift to (309) 566-5688.
Stock/Securities
To make a
gift of securities today, please contact the foundation office (309)
566-5666 to arrange a transfer.
Corporate/Foundation Gifts
If your
company would like to invest in the future of INI Foundation, please contact the Foundation office at (309)
566-5666.
Matching Gifts
Many
contributors multiply their support to INI Foundation by making use of their employer's matching gift
program. We are grateful to these companies and foundations
for their generosity. We encourage you to ask if you employer
offers such a program. Please contact your employer's human
resource director for the necessary form to send with your gift.
Memorial Gifts
Memorial
gifts are a lasting remembrance of a special person who has touched
you during his or her life. INI Foundation will acknowledge your gift by sending an
appropriate note to the family or friend in whose name the gift was
made. The amount of your gift will not be mentioned. A

Tribute gifts
