Case Management
Many injuries and illnesses require treatment from a complex and comprehensive
team. Patients may be cared for in a variety of Saint Francis Medical Center
departments or at a variety of facility sites. Case management coordinates the
care. The goal of this service is to ensure that the patient and family receive
the highest quality of care in a timely manner, while using resources efficiently
(including coordination with payers).
Questions or concerns during rehabilitation treatment can be forwarded to the
case manager for assistance.
Physical Therapy
The purpose of physical therapy (PT) is to promote physical fitness
and well being, improve movement and function, and relieve pain
and prevent injury. The therapeutic properties of exercise, heat,
cold, electricity and massage are used to improve circulation,
endurance, encourage return of motion, strengthen muscles and
train or retrain patients to perform the skills of daily living.
In addition, physical therapists prescribe home programs, provide
family training, and recommend assistive devices, orthotics and
prosthetics.
Rehabilitation
Nursing
Rehabilitation nurses use their skills in Rehabilitation Services' Comprehensive
Inpatient Rehabilitation Unit. The rehabilitation nurse is a specialist who
uses many unique skills to help make the most of patient's
abilities. The nurse works with patients and family to give support,
counseling, and teaching about
daily activities such as eating, grooming, bathing, dressing, bowel, and bladder
care, skin care, mobility, medication and overall safety.
Rehabilitation nurses help patients develop a plan of care designed for patients' needs. They work with the patient and the rehabilitation team to prepare patients for discharge.
Social Work
Social workers assist with adjustment to life changes and discharge
planning. The social worker provides ongoing supportive
counseling and information regarding
treatment and discharge planning to patients and families. Our social workers
may also assist patients in accessing financial resources, filling out paper
work and pre-authorizing services.
Social workers can provide referrals to various services and community resources which facilitates a smooth transition process throughout the continuum of care. Referrals may include:
- Home Health
- Outpatient Therapy
- Home-delivered meals
- Skilled nursing facilities
- Vocational rehabilitation.
